- The names that are to be filled in should be part of the professional certification.
- Forms number four and six should be written in accordance with S0123-0001 or P0123-0001.
- Make sure the number you are using is correct.
Documentation
1. You are required to attach documents for all professional information, work license and birth certificate.
2. Certificate sent make sure it is not larger than 2 MB.
